What vitamin B12 does
Vitamin B12, also called cobalamin, takes part in three processes that cannot afford to fail. The first is forming red blood cells: without enough B12, the bone marrow produces large, dysfunctional cells, leading to a specific type of anaemia (megaloblastic) that explains the fatigue and paleness typical of deficiency. The second is maintaining the nervous system: B12 is needed to preserve myelin, the sheath that coats the nerves, and its prolonged lack can cause tingling, balance problems and, in advanced cases, neurological damage that does not always fully reverse. The third is DNA synthesis, that is, copying genetic material during cell division. That a single vitamin sustains the blood, the nerves and cell replication explains why its deficiency, when it comes, shows up on so many fronts at once.
The symptoms: silent and easy to miss
The big problem with B12 is that its deficiency advances slowly and its signals are non-specific, so it is easy to blame something else. The most common:
- Fatigue, weakness and paleness, from the anaemia it causes.
- Tingling or numbness in hands and feet, and balance problems (neurological signals).
- Difficulty concentrating, memory lapses or "brain fog".
- Mood changes, irritability or apathy.
- A smooth, red, sore tongue (glossitis) and sometimes mouth ulcers.
Who is most at risk
B12 deficiency has two main causes: eating little or no animal sources, or not absorbing it well even when you eat it. The highest-risk groups are:
- Vegans and strict vegetarians: without animal sources or enough fortified foods, deficiency is a matter of time.
- Older adults: with age, the ability to absorb B12 from food declines, even for meat-eaters.
- Long-term use of metformin (diabetes) or antacids/proton pump inhibitors: both reduce absorption.
- Digestive surgery (gastric bypass, resections) or conditions such as atrophic gastritis or pernicious anaemia, which damage the absorption mechanism.
Where B12 is: animal only (and fortified)
Here is the key quirk of this vitamin: there is no reliable natural plant source. B12 is made by bacteria, and in the food chain it reaches us through animal foods. The best sources are organ meats (liver), shellfish (clams), oily fish (sardines, salmon), meat, eggs and dairy. Neither algae, nor unfortified yeast, nor fermented foods provide active B12 in useful amounts: it is a persistent myth worth debunking. The only valid plant route is FORTIFIED foods — some plant drinks, breakfast cereals or fortified nutritional yeasts — and even then you have to eat enough, consistently, for them to count. For everything else in a plant-based diet there is no problem; B12 is its one nutritional Achilles heel.
When (and how) to supplement
Supplementing B12 is not a biohacker whim: for certain groups it is a basic medical need. If you are vegan, it is mandatory, full stop. The reason is not just precision, but safety: high-dose folic acid (common in multivitamins) can correct the B12 anaemia on the test while the neurological damage keeps advancing silently. The right approach, if you suspect it from symptoms or from belonging to a risk group, is to ask your doctor to measure B12 (and, if needed, homocysteine or methylmalonic acid, which detect functional deficiency earlier).
